Root Cause Analysis

Have you tried fixing a problem or improving a process only to find that things just don’t get that much better? Maybe you only addressed a symptom of the real, underlying problem. Root Cause Analysis (RCA) is an organized, effective, and often revealing way to explore the various causes of problem or substandard performance. RCA results in the identification of the one or few “root causes” that, if addressed, will lead to breakthrough improvement. A principle tool used in RCA is the Cause & Effect Diagram (aka, Fishbone Diagram, Ishikawa Diagram), which visually depicts the various “causes” of a particular “effect”. Users can identify how the causes may be related, and which ones have more influence.
